JOB DESCRIPTION:
Be part of making a difference as you will be providing continuity of care through the lifespan of our patient population, primarily diagnosed with IDD
Strong focus on providing quality care, with adequate time given to see patients and administer care, as well as document progress
As a primary care provider, you will be integrated amongst the community, participating in community events and involved throughout the patient's lifespan

QUALIFICATIONS:
Comfortable providing care to a patient panel within the full lifespan, child to adult
Desire and passion to work with the underserved
BE/BC
NY State licensed
