Job Responsibilities:
Obtains and documents health history, performs physical examinations, educates patients on preventative health measures for both children and adults
Knowledge of DSM-IV diagnostic attributes
Documents and maintains patient's records of care on Health Center EMR
Develops and institutes appropriate treatment plans per protocol. Treatment will include the care of acute and chronic medical problems as well as contraceptive services
Participates in multidisciplinary teams to improve patient care processes and outcomes

Qualifications:
The candidate must be able to work independently and autonomously, as there will be occasions without supervision. 1 year experience REQUIRED, 2 years preferred.
Active DE NP and DEA licenses.
Spanish bilingual PREFERRED but not required
