DETAILS:
Scheduled Post-Discharge visits
Provides primary care diagnosis and treatment in a family practice clinical setting to patients recently discharged from an inpatient or emergency department setting.
Counsels and educates patients regarding treatment and refers to ancillary and support services.

Will rotate between all of our primary care clinics (Coeur d'Alene, Post Falls, Rathdrum, and Kellogg)

QUALIFICATIONS:
Minimum 2-4 years of experience directly related to the duties and responsibilities specified
State of Idaho RN Licensure, DEA, and Idaho Controlled Substance or ability to receive licensure required.
BLS certification or must obtain within 90 days of hire
