DETAILS:
Reaches out and engages clients with appropriate concern for their behavioral health care needs
Coordinates medical care with PCP for chronic and acute non-behavioral health conditions
Advises and counsels clients about their physical and mental health; coordinates care with case management and health education as clinically indicated
Assists with the identification, counseling, and care coordination, with case management and health education, of mental health and substance abuse problems
Works in a non-traditional environment; e.g., the non-clinic facility where homeless people congregate
Provider will be servicing the men who live in the shelter initially, and patient panel will expand as role expands
