DETAILS:
Obtains health histories and performs physical examinations and screenings, including testing for HIV on-site
Reaches out and engages clients with appropriate concern for their health and other needs
Conducts health screening services
Works in a non-traditional environment; e.g., the non-clinic facility where homeless people congregate
Makes every effort to provide and assure ongoing medical service, including a continuum of care for persons with HIV/AIDS
Provider will be servicing the men who live in the shelter initially, and patient panel will expand as role expands
