JOB DESCRIPTION:
Works as part of the medical provider team, providing medical services to the patients of the community health center.
Provide primary care services as licensed independent practitioners
Actively participate in the implementation and evaluation of the Patient-Centered Medical Home using team-based care to achieve improved health outcomes as well as improved patient experiences.
Be willing to provide services to our school based partnerships within our mobile units

QUALIFICATIONS:
SC license
1 to 2 years of experience preferred but willing to accept RN experience
Current MAT certification or willingness to obtain
