Join four Geriatricians who support nursing home rounds, along with 7 APPs and 3 full-time nursing staff
Primary responsibilities include overseeing patient care at area nursing homes in post-acute care, long-term care, and assisted living/memory care
Work in close collaboration with the APP team to manage patients' needs

Family or Internal Medicine candidates with significant Geriatrics experience or a fellowship-trained geriatrician candidate are welcome to apply
Hospital practice is not required

Physicians will be on call for their nursing home patients during the workday week as needed; however, APPs handle the majority of calls
Physicians rotate "MD Backup Call" between the 4 Geriatricians, serving as a resource to APPs as needed for nights/weekends
