Monday: 7:30 - 6:30 (extended hours rotated by providers on staff), Tuesday through Friday: 8 - 5. Each provider gets a half-day per week for admin time

Seeking a compassionate and patient centered Primary Care Physician. This position reports directly to the Chief Medical Officer and plays a vital role in delivering high-quality, comprehensive care within our FQHC.
The PCP will serve as both an independent and collaborative member of the healthcare team, providing full scope primary care services while supporting our mission to improve access to care for the communities we serve.
Key Responsibilities
Assess, diagnose, plan, and provide comprehensive primary care services to patients across the lifespan
Deliver care in a courteous, efficient, and professional manner consistent with FQHC standards
Provide clinical guidance and supervision to medical assistants, nursing staff, and medical students as applicable
Qualifications
Eligible for and able to obtain medical licensure in the state of Indiana
BE/BC in Family Medicine
Current CPR certification
Ability to meet credentialing requirements of affiliated hospitals
