What You'll Do
Serve as primary care physician for PACE participants (55+)
Provide preventative, acute, and chronic care with a focus on aging with dignity
Collaborate with the Interdisciplinary team to coordinate holistic care plans. The interdisciplinary team includes on-site social work, physical and occupational therapy, a registered dietician, a day center coordinator, home care, transportation, and more.
Build trusted relationships with participants and their families

Requirements:
MD/DO with interest in geriatrics
BE/BC in IM, FM, or Geriatrics
Active CA license + DEA
Meets 42 CFR Part 410.20
Minimum 1 year experience with frail/elderly population
