DETAILS:
Home site with willingness to travel within 30 miles (four sites)
Assess physical and psycho-social health status through medical history and physical examination
Care team acts as "coaches" who help their patients get healthy, stay healthy, and get the care and services that are right for them.

QUALIFICATIONS:
NY License REQUIRED
3 yrs experience in Community Health PREFERRED
Hospital Privileges PREFERRED
