Northeast Florida Health Services, Inc., a Federally Qualified Health Center (FQHC), is seeking a fulltime Family Medicine Physician (MD/DO) to provide high-quality, patient-centered care in our community health centers.

Responsibilities
Provide comprehensive primary care, acute care, and chronic disease management.
Deliver preventive care and health screenings tailored to individual patient needs.
Collaborate within a multidisciplinary care team to address overall medical and social needs.
Maintain accurate, timely documentation in the electronic health record (EHR).
Participate in quality improvement and population health initiatives.
Ensure compliance with HRSA, HIPAA, and state and federal regulations.

Qualifications :
FL License
Experience in FQHC, or Community Health
Spanish Bilingual a Plus
