Position will spend two days at Chicago Heights location, two days at South Holland, and the remaining week day will be telemedicine.

10 Hour Shifts 4 Days per Week or 8 Hour Shifts 5 Days per Week
Provide continuous, comprehensive primary care for infants, children, youth, and adults according to current standards of care
Make diagnoses and prescribe medication as necessary

QUALIFICATIONS:
IL License REQUIRED
