Primary care hours are- Mon, Wed, Fri 8 am - 5 pm, Tue, Thu, 8 am - 7 pm, and one Saturday per month

100% outpatient; Monday - Friday
Patient-Centered Medical Home Model housed in a large multi-specialty center which includes Adult Medicine and Pediatrics as well as Endocrinology, Rheumatology and Dermatology
Physician and APP Partnership exist to manage a panel of patients
Call is 1:7 telephone only, and supported by nurse advisory line
