DETAILS:
Home site with willingness to travel within 30 miles (four sites)
Provides patient education including diagnosis, treatment plan, and prognosis
Care team acts as "coaches" who help their patients get healthy, stay healthy, and get the care and services that are right for them.

QUALIFICATIONS:
NY License REQUIRED
3 years experience in Community Health PREFERRED
