DETAILS:
Performs case management and counseling activities related to psychosocial needs, affecting the health and well-being of patients
Assess the patient's psychosocial needs and determine appropriate treatment/referral needs
Assists patients in accessing prescription cost assistance through indigent programs
Acts as an advocate for patients when interacting with community agencies

QUALIFICATIONS:
Licensed counselor/therapist in NY
Spanish speaking preffered
