Additional Information:
- As the Behavioral Health Therapist you will identify, triage and manage patients with behavioral health and substance use barriers within the primary care setting.
- Assess patients with mental health, psychological, and/or substance use issues, create care plans, and manage care plans with a care team of primary care providers.
- Conduct patient intakes, focused on diagnostic and functional evaluation, making appropriate mental health treatment recommendations to the primary care providers, and establishing a positive rapport with patients.
- Provide individual, family, and group counseling.
- Provide episodic counseling services to patients in acute crisis.
Required Training & Experience:
- Must possess a current CPR certification.
- Must be licensed as an LMSW or LPC
- CAADC is preferred but not necessary.
