Additional Information: This role is essential in supporting the Chronic Care Management (CCM) and Advanced Primary Care Management (APCM) Programs. As a Registered Nurse (RN), you'll help patients with chronic conditions receive the coordinated, personalized care they need to achieve better health outcomes. Build meaningful relationships with patients while collaborating with providers, specialists, pharmacies, caregivers, and community resources to ensure seamless care. Key responsibilities include developing and updating individualized care plans, conducting patient outreach, reconciling medications, documenting care in the EMR, following up on referrals and hospital discharges, and identifying preventive care needs. You'll also help ensure compliance with CMS guidelines and quality initiatives while making a meaningful difference through compassionate, patient-centered care.
