JOB DESCRIPTION:
-Be part of making a difference as you will be providing continuity of care through the lifespan of our patient population, primarily diagnosed with IDD
-Strong focus on providing quality care, with adequate time given to see patients and administer care, as well as document progress
-As a primary care provider, you will be integrated amongst the community, participating in community events and involved throughout the patient's lifespan
