Bay Area Community Health was formed in 2020 off the strength of southern Alameda County’s Tri-City Health Center and Santa Clara County’s Foothill Community Health Center. Formed during the middle of the COVID-19 pandemic, Bay Area Community Health (BACH) brings more than 70 years of combined service to an area that stretches from Union City to Gilroy.

Before forming BACH, Tri-City Health Center served the Fremont community for 50 years, becoming a central force for providing health services to all in southern Alameda County.

Foothill Community Health Center began servicing low-income families and individuals in East San Jose in 1996 and the greater Santa Clara County region in 2011.

Both organizations bring decades of service, relationships, and professional staff to Bay Area Community Health. BACH serves more than 100,000 people, who rely on high-quality healthcare services, regardless of their immigration status, ethnicity, disabilities, or ability to pay.

Located on the southeast side of the San Francisco Bay, Fremont is a city of over 214,000 people with an area of 90-square miles, making it the fourth most populous city in the Bay Area and California's 15th largest city. Fremont is located within Alameda County. Fremont is ideally situated on the southeast side of San Francisco Bay in the northern part of Silicon Valley. It is easily accessible via Interstate Highways 880 and 680 with a Bay Area Rapid Transit (BART) station within walking distance from Downtown. Located within the City Center, downtown has views of Mission Peak and is a mile away from Central Park and Lake Elizabeth, a major recreation spot.

Today, Fremont is one of the most ethnically and culturally diverse cities in the Bay Area. Fremont’s top schools, well-educated population, and high household income make it an ideal place to live and work.

With its moderate climate and its proximity to major universities, shopping areas, recreation and cultural activities, employment centers, major airports, and the Bay Area Rapid Transit system, Fremont captures metropolitan living at its best.

Recreation in the area includes several community parks, historical sites, Central Park with Lake Elizabeth, Aqua Adventure Waterpark, a sports complex, tennis center, Fremont Park Golf Center, Olive Hyde Art Gallery, three program centers, and four community centers.

The Fremont area features a variety of things to do and wonderful places to explore. Whatever your age or interests, there’s something to see in and around Fremont. The Cultural Arts Complex in the Gary Soren Smith Center for the Performing Arts brings music, theater and dance performances to the Fremont area. The Fremont Symphony Orchestra performs concerts from September to May. The Fremont Art Association Gallery presents exhibitions of many local artists. The Olive Hyde Art Gallery offers gallery exhibits introducing an array of traditional and contemporary art.

The Fremont area is also replete with much-anticipated community events. Just a few examples include the annual Fremont Festival of the Arts during the first week in August, the Farmers Markets in Centerville and Irvington and the Alameda County Fair in Pleasanton.

Fremont offers a wide range of recreational facilities and activities including more than 50 community parks, several community recreational centers, sports facilities, an ice rink, acres of preserved open space and miles of trails. The 450-acre Central Park, with 83-acre Lake Elizabeth at its center, provides tennis courts, sailing and boating, softball and soccer fields, picnic areas, volleyball courts, playgrounds, a skate park, a dog park, a nature center and Aqua Adventure Water Park. A senior citizen center, animal shelter, and Fremont Mail Library are also located in or adjoining the park.
