    DETAILS:
    - You will participate as a member of the PACE Interdisciplinary Team in the care of high-risk older adults living in the community
    - Will act as the leader of the Clinical Team whose primary mission is to assist older adults to remain living in the community for as long as it is safe
    - Monday-Friday scheduling, with a possibility for a 32-hour workweek or 40-hour workweek
    - Participates in the PACE hospital rounding system as well as nursing home visits
    - Evaluates the needs of the participants and develops treatment plans in concert with the other members of the Interdisciplinary Team
    - Experience working directly with Geriatric patients required
