Compensation:
- $60,000 - $70,000

DETAILS:
- Work effectively in a patient-centered, integrated care model in a variety of clinical settings
- Participate in shared treatment sessions with providers and patients
- Participate in integrative care team meetings to coordinate care
- Construct, document, and execute robust treatment plans using the biopsychosocial model

QUALIFICATIONS:
- Master's Degree REQUIRED
- LCSW, LMHC, or LMFT License REQUIRED
- IN License REQUIRED
