DETAILS:
- Works as part of the multidisciplinary care management team which includes a Registered Nurse Care Manager and Community Health Worker
- Focuses on engaging patients in their own care through consistent assessment, measurement of progress, outreach, education, and referral management efforts
QUALIFICATIONS:
- MA in Social Work or similar field REQUIRED
- TX License REQUIRED
- Bilingual (SPAN-ENG) PREFERRED
- Experience PREFERRED
